All pregnant women enrolled in managed care through PHPs will continue to receive a coordinated set of high-quality clinical maternity services through the Pregnancy Medical Program PMP. This program will be administered as a partnership between PHPs and local maternity care service providers defined as any provider of perinatal services.

A key feature of the program will be the continued use of the standardized screening tool to identify and refer women at risk for an adverse birth outcome to the Care Management for High-Risk Pregnant Women CMHRP program, a more intense set of care management services that will be coordinated and provided by Local Health Departments LHDs.

Together, these two programs will work to improve the overall health of women and newborns across the state. The Medicaid program currently offers a set of care management services for at-risk children ages zero-to-five.

The program coordinates services between health care providers, community program and supports, and family support programs. Medicaid beneficiaries of all ages may receive Long-Term Services and Supports LTSS program services for a short or extended time in an effort to support their goal of regaining or maintaining maximum health and independence when living with a chronic illness or disability. You must complete a subsidy application form along with the required documents on the checklist and return it to our Child Care Management Program office.

Forms may also be requested by calling our Montgomery office at Family is the basic family unit consisting of an adult and his or her spouse including common law , children under 18 years of age, and minor parents under 18 years of age and their children, related by blood, marriage, or adoption, who are residing in the same household.

Box Opelika, AL Box Anniston, AL CareCheck: is the term for required child abuse and State Police background clearances for relatives who care for children whose parents participate in the subsidized child care program. Relatives who care for three or fewer children, not including their own children, are not required to have a state license but can receive subsidized child care funding. Relatives who participate as providers for the subsidized child care are required to complete CareCheck.

CareCheck is the Department of Human Services program that requires State Police criminal history and child abuse background clearances for all relative providers. In addition to CareCheck, relatives must obtain Federal criminal history clearances. Relatives must complete CareCheck and Federal criminal history clearances in order to be eligible to be paid through the Subsidized Child Care Program.

Relatives are defined as grandparents, great-grandparents, aunts, uncles, and siblings. All relative providers must be 18 years of age or older and live in a residence separate from the residence of the child for whom they will provide care. Increase access to health care.
